Patient FAQ’s

What do I need to know prior to my first visit?

  • Wear loose, comfortable clothing (tank tops for shoulders and shorts for the lower body).

  • Plan to arrive five minutes prior to your appointment. Please take a seat in the front waiting room area when you walk into Fitness Hero. Your therapist will be with you shortly.

How does payment work?

We are currently in network with BCBS PPO; we are a NON-PAR provider of Medicare, and offer self-pay rates. You will receive a statement every two weeks for your patient balance. We appreciate prompt payment. Self-pay patients and dry needling will be charged at time of service.

BCBS PPO Patients: Please submit a copy of your insurance card, front and back, to admin@endurance-pt.com

Medicare patients: Endurance Physical Therapy is a non-par provider with Medicare. As a non-par provider, Endurance Physical Therapy will bill a Medicare patient for their physical therapy sessions based on a Medicare fee schedule at the time of service. Endurance Physical Therapy will also bill Medicare for the patient at the time of service. The Medicare patient should receive reimbursement (in approximately one month) from Medicare and their secondary insurance. This reimbursement should cover approximately all but 15% of what was charged from Endurance Physical Therapy.

Endurance Physical Therapy is not responsible for the reimbursement payment. It is the patient’s responsibility to follow up with Medicare and their secondary insurance with any questions. Please submit a copy of your Medicare card and secondary card to admin@endurance-pt.com

Self-Pay Patients:

The prices for self-pay are:

  • Initial Evaluation: $175

  • Follow-Up Treatment Sessions: $150

·       If you have a plan outside of IL BCBS, please contact the number on the back of your card to find out if you have Out-of-Network coverage for physical therapy. For patients who would like to be seen at a self-pay rate, we are more than happy to provide you with a monthly invoice/receipt for you to submit to your insurance.

Where is Endurance Physical Therapy located?

The clinic is located inside Fitness Hero Wellness Center at 2909 MacArthur Blvd, Northbrook, IL 60062.

 

What happens if I cancel my appointment?

Your appointment time has been reserved just for you. Keeping all your appointments is an integral part of your recovery. If you do need to cancel an appointment, please be aware of our requirement of a least 24-hour notice. Failure to cancel your appointment with more than 24-hour notice will result in a $75.00 fee. Insurance will not pay for this fee.
